Psycho-Social Rehabilitation

 Psychosocial Rehabilitation (PSR) is designed to help adults with behavioral health challenges increase their ability to live independently and minimize their need for ongoing professional intervention.  We help people set and pursue their personal goals and implement steps to better manage their illness and their everyday lives.  Enhanced skills and increased feelings of confidence, nurtured within a caring environment, help adults with behavioral health needs maintain the dignity, respect, and sense of pride that enable them to become active, productive members of their communities.  Success in the real world comes, in part, from knowing how to manage responsibilities and learning how to make the right choices. Our PSR programs are designed to promote these building blocks of success.

Substance Abuse Intensive Outpatient Program (SAIOP)

 

Prestige Services Group LLC SAIOP involves a series of structured individual, family, and group activities and services provided on an outpatient basis designed to assist adult and adolescent clients with addiction disorders to begin recovery and learn skills for recovery maintenance.  SAIOP also informs the client about benefits, community resources and services, and services; assists the client in accessing benefits and services and monitors the provision of services.  SAIOP provides education on both wellness and recovery.

Participants may be residents of their own home, a substitute home, or a group home setting; however, SAIOP services will be provided in a setting separate from the client’s residence in locations that meet the needs of the persons being served.

SAIOP is offered at times that maximize the ability of the person served to participate in the program SAIOP meets a minimum of 9 hours per week over a minimum of 3 days per week with no more than 2 consecutive days without services being offered equally out to be at least 3 hours each day.  Each day the client is in treatment, the person will be encouraged to attend at least one recovery support group of their choosing.  Each client will be alcohol and/or drug screened as clinically indicated.  Monthly PersonCentered Plan (PCP) updates will take place to review and document necessary changes to one’s plan.  Each client is also provided necessary case management services as listed within one’s PCP.

SAIOP is typically provided to each client for a period of three months.

SAIOP services include the following (note service components are provided based off one’s goals in their plan):

  • Individual counseling and support
  • Group counseling and support
  • Family counseling, training, or support
  • Biochemical testing to identify recent drug and or alcohol us (UA and BAC)
  • Relapse prevention (including community supports)
  • Life skills training
  • Crisis contingency planning
  • Disease management
  • Treatment support activities adapted specifically for person with physical disabilities or persons with co-occurring disorders
  • Case management activity to arrange, link, coordinate, or integrate multiple services, assessment and re-assessment of the client need for services

The primary goal of SAIOP is abstinence from addictive substances.  The secondary goals and outcomes may include sustained improvement in overall health, better relationships with family and friends, improvements in finding and maintaining work, reduced depression and anxiety (if present), and reduced risk of relapse in the future.  SAIOP will be tailored to fit each client's needs by developing a Person-Centered Plan which helps one identify goals, needs, and preferences in one’s treatment and life. 

Substance Abuse Comprehensive Outpatient Treatment

 Substance Abuse Comprehensive Outpatient Treatment (SACOT) is a form of recovery treatment that is designed to help those overcome their battle with substance abuse and mental health issues. This form of outpatient treatment is similar to an inpatient treatment program in that treatment is fully immersive and intensive, yet it differs in that clients can return to their own homes at the end of each day. A Substance Abuse Comprehensive Outpatient Treatment program is beneficial for those who are facing addiction as well as underlying mental health challenges. By meeting 5 times per week, clients have the ability to work through their challenges and learn effective coping skills to overcome their addiction and maintain long-term recovery. 


 

During Substance Abuse Comprehensive Outpatient Treatment, clients will visit our facility 5 days per week, Monday-Friday from 10 a.m. to 2 p.m. The average day during SACOT includes:

  • Individual and Group Therapy
  • Educational course on relapse prevention & life skills
  • Periodic evaluations
  • Music & art therapy
  • Yoga & Meditation
  • 12-Step immersion
  • Cognitive Behavioral Therapy
  • Trauma & grief counseling

HIV Case Management

 

HIV Case Management is a client-focused strategy that provides cost-effective, medically necessary services to enhance beneficiary health status and level of functioning.

The goals of HIV case management are to:

  • Improve an eligible beneficiary’s access to a wide range of appropriate services
  • Promote continuity of care by coordinating service delivery arrangements
  • Enhance a beneficiary’s health status and level of functioning
  • Promote efficiency by reducing or containing the overall cost of services

HIV case management is not intended to be an ongoing service. It is a short-term, goal-oriented service tailored to meeting the specific immediate needs of the beneficiary. It is designed to empower clients through education, referrals and facilitating access to care, assisting clients to gain the tools needed to be their own advocates and navigate the health care and social services systems to improve health outcomes.

On-site and remote CARF consulting services are provided in the U.S. 

Mock Surveys. Onsite or remote reviews and mock surveys are often the best way to prepare the organization for an actual CARF survey.  This can be an efficient way to become familiar with the survey process.  Mock surveys are useful when preparing for initial accreditation, when a new program is added under a currently accredited organization, or in preparation for a re-survey.  Mock surveys are individualized for each organization, and can include:

  • Pre-survey contact and review of non-confidential materials prior to the mock survey
  • Orientation Conference and tour
  • On-site or remote review of policies and procedures, administrative plans, program plans, clinical forms, administrative and program documents, and client records, as well as interviews with governance, leadership, funders, staff, and clients
  • Exit Conference with review of draft mock survey report
  • Debriefing of leadership and selected personnel
  • Finalized report with recommendations and consultations
  • Five days of consultation after receiving the Mock Survey Report.

Personnel Training. Personnel training for an in-depth review of the CARF BH or CYS Standards Manual typically includes the following, and can be modified to meet the needs of the organization:

  • changes to the Standards Manuals and standards for which recommendations are often given during on-site CARF surveys
  • a general review of all standards in Sections 1-5 that are relevant to programs for which the organization is seeking accreditation
  • discussion about the Intent and Examples statements as relevant
  • how to respond to questions from the CARF surveyors during the on-site review

Policy and Procedure Development and Review.  This service includes writing new policies and procedures as well as reviewing existing documents to ensure continued relevancy and conformance to CARF standards.  The review also includes identification of inconsistencies and redundancy.  The frequency of reviews is determined based on the needs identified by the organization and as suggested by the CARF Consultant.

Document Development and Review.   This service includes report writing, document development, proof-reading, editing, and formatting of documents as well as annual updates to ensure conformance to CARF standards.  When an organization contracts for ongoing review of documents, the frequency of reviews is determined based on needs identified by the organization and as suggested by our CARF Consultant.  Many documents are required to be reviewed annually (e.g., program plans, strategic plan, risk management plan, etc.).

  • Administrative Documents include:  The six administrative plans (i.e., Cultural Competency & Diversity Plan, Strategic Plan, Risk Management Plan, Technology Plan, Accessibility Plan, Performance Analysis & Improvement Plan); ethical codes of conduct; job descriptions; emergency procedure drill forms; grievance forms; requests for reasonable accommodations; quarterly billing audits; and other documents.
  • Program Documents include: program plan(s); screening form; client orientation handbook and checklist; assessment form; treatment plan; transition plan; discharge summary; quality records review form; and other documents.

Quarterly Records Review.  Due to lack of time and/or staffing, it is often difficult for organizations to conduct the required quarterly records review of a representative sample of open and closed charts (e.g., at least 10% of each).  The CARF Consultant is available to provide this on-site or remote service on a quarterly (or more frequent) basis.  Written findings will be provided to managers for incorporation into training programs.

Survey Design, Administration, and Analysis.  CARF-accredited organizations are expected to continually focus on the expectations of the clients, personnel, and other stakeholders. This includes soliciting, collecting, and analyzing input from clients and staff in order to implement changes in services to meet the identified needs. The leadership is also required to use the information in program, financial, resource, and strategic planning as well as for performance improvement and organizational advocacy. This component of continuous quality improvement can be challenging for many organizations. The CARF Consultant can simplify this task by designing and administering the surveys, providing a graphic analysis of the results, and write an outcome report. On-line and/or paper surveys can be designed to meet the unique needs of each organization.

Pre-Survey Preparation and Organization.  Consulting services can be especially helpful in the year prior to the survey or re-survey.  We can assist in the organization of documents and other materials for easy access of the CARF surveyors.  When a surveyor cannot find the required information within a reasonable time, assistance is sought from the organization.  When evidence of conformance to the standards cannot be produced by the organization, recommendations can be given.  Contracting with a CARF Consultant to prepare for the on-site survey can reduce the number of recommendations received.

Quality Improvement Plan (QIP).  In order to be useful, a good QIP contains interventions that effect actual and continuous change.  For example, a common problem for organizations is that treatment plans are often not consistently measurable, achievable, and time specific.  An ineffective QIP action might state: “Direct-service staff will receive training about how to write objectives that are measurable, achievable, and time-specific.”  Although at “face value” this appears to be an effective intervention, training of staff often does not mean a direct translation to actual and consistent change.  Contact us to learn more about how the CARF Consultant can help write and implement effective QIPs.
